Physical Therapy After ACL Surgery – What to Expect

After an ACL surgery, the recovery process can seem daunting. However, our trained professionals will be with you every step of the process and make sure you know what to expect. While the recovery process will vary for every individual, a general timeline is as follows:

First Week Post Surgery

During this time frame, you will be required to stay in bed with your knee elevated. You will use a continuous passive motion machine to help reduce pain, alleviate stiffness and swelling, and promote healing. You will also perform physical therapy exercises to improve knee extension and flexion. Your first follow-up appointment will also be during this time. The goal is to reach 120 degrees of flexion.

Second Week Post Surgery

During the second week, you will continue to perform knee exercises with goals to continue increasing flexion to 135 degrees and be able to lock the knee straight while standing. Unweighted exercises and towel stretches will be incorporated into your daily routine. Your second follow-up appointment will also be during this time.

Weeks Two to Four Post Surgery

At this time, you will continue to improve your range of motion with physical therapy exercises and can begin doing lightly weighted exercises if allowed by your physical therapist. Your physical therapist will design a custom plan for your rehabilitation and will closely monitor and modify this depending on your progress. Your third follow-up appointment will be sometime during this time frame.

Months Two to Twelve Post Surgery

During this time, you will move towards your original (or improved!) level of activity. Your physical therapist will closely monitor your range of motion, swelling, pain levels, strength, and healing process. Based on this data, they will advise the level and pace at which you may return to different activities. You will also have follow-up appointments every 1 to 3 months during this time frame to evaluate for any complications.
