One2Four
Level 1: Total dependence to maximum assist
Level 2: Maximum assist to independent home
Level 3: Independent home to high performance
Level 4: High to ultimate performance

Is there a link between the person that cannot climb stairs due to hip pain and the same person moving smoothly through another yoga class? How does one go from being unable to sleep due to nagging shoulder pain to lifting a smiling grandchild high overhead? What path does a person follow from being unable to walk due to a serious knee injury, run again?

One2Four TM is a link, a method and a path for people to travel in order to do activities that for a moment seem impossible but with the proper guidance and time, obtainable.

One2Four TM is based upon the fact that all people fall somewhere along a health continuum from total disability to ultimate physical performance/function.  In between these extremes of the health continuum are four levels of physical performance where all people are found.  These levels go from a level One, of being dependent on others, to a level Four, in which are people who are striving for excellence in physical performance.   Each of the people in the initial examples was at one functional level and ultimately able to move to another, higher level of function.

How is this done?

One2FourTM Patient/Client

In these cases a Twin Boro Physical Therapist determined for each individual what level of the 4 functional levels within One2FourTM the person was at; one, two, three or four.   Then the goals for each person were established.  After an examination by the Twin Boro Physical Therapist a plan of care was developed to guide the person from the initial low level of function, one or two, to higher functional levels, three or four, that was able to improve the quality of life for each of these people.  What was done for them is done for others each and every day.

Who is a candidate for the Twin Boro One2FourTM program?

Any person who is not satisfied with his/her present level of function and desires to perform at a higher functional level.   One2FourTM will help a person at level one (1), who due to pain and/or weakness is dependent on others for the most basic daily tasks, become able toperform all daily tasks without trouble which is characteristic of level three (3).   Simply put, One2FourTM can help each person who strives to obtain higher physical performance/functional levels.

Healthcare continuum:

Level 1

Level 1 characterized by the individual being dependent of others for activities of daily living (ADL).

Level 2

Level 2 characterized by the individual requiring assistance with ADL.

Level 3

Level 3 characterized by the individual being independent in basic ADL however quality of sports performance is limited to low to moderate intensity activities in unidirectional planes of motion.

Level 4

Level 4 characterized by the individual who is working on improving athletic performance at high intensity in multidirectional planes of motion whether or not the individual is returning from an injury or illness.

LEVEL 1 LEVEL 2 LEVEL 3 LEVEL 4
Level of Funtion Assistance for most ADL ADL limitations Independent basic ADL Sports Performance
Exercise Intensity Low (passive to assistive) Low to moderate (assistive to active) Moderate ≥ high Moderate ≤ HIGH
Primary planes of motion Unidirectional Unidirectional to assistive multidirectional Unidirectional ≥ Multidirectional Unidirectional ≤ Multidirectional

"Twin Boro Piscataway/South Plainfield is great! I was treated for occipital neuralgia and posture issues. Aside from performing wonderful treatment, everybody there is super friendly and accommodating (you will always be greeted with a smile) and also very focused and helpful! Bri is a knowledgable and experienced physical therapist who is not only a great PT, but also one of the friendliest people you will ever meet. Her pacing with my various exercises was perfect, as she pushed me, but not to any point that was uncomfortable for me. I enjoyed the variety of exercises and treatments, from heat and stim to exercises with weights and stretch bands. I thoroughly enjoyed going in 3 times a week to be treated by Bri. It is clear that she cares about and wants the best for her patients, with her passion and expertise for physical therapy! My posture has gotten much better and the headaches and pain I used to experience have completely disappeared after 3 months at Twin Boro. 5/5 I would recommend Twin Boro Piscataway/South Plainfield to anyone seeking physical therapy."

“After my bilateral hip replacement I had a choice where to do my outpatient therapy. I chose to go to Twin Boro because of the positive experience I had there previously. Every staff person I worked with knew what they were doing, were friendly and contributed to a very positive experience for me.”

Wiken

My experience with Twin Boro after my total knee replacement has been exceptional. They made all my appointments to fit into my schedule, explained all my exercises to me and made me feel very confident that I would be taken care of.”

Patricia

The Bridgewater Location is excellent! Every single staff person is on point. From day one when I called to schedule my first appointment and throughout my treatment staff were helpful, professional, and overall a caring and compassionate team. Their professionalism and caring was evident not only in my treatment, also in the treatment of other patients. Being in a room with multiple treatment stations you cannot help but notice the care received by other patients. My ankle is 100% better and if I ever need PT again I would not hesitate to return. I was always a part of the therapy process, if I had questions they were answered. If I wanted to improve my ankle health at home I was provided clear instructions for exercises. Staff were skilled and knowledgeable. I highly recommend this location to family and friends.
