Physical therapy exercises for achilles tendonitis

Achilles tendonitis is caused by acute inflammation of the Achilles tendon. The Achilles tendon connects the calf muscle, made of the gastrocnemius and soleus, to the heel bone, or calcaneus. When this tendon becomes inflamed, patients often feel pain along the back of the ankle and heel.

Achilles tendonitis can be insertional or mid-portion. Insertional Achilles tendonitis tends to lead to pain where the Achilles connects to the heel bone, while mid-portion Achilles tendonitis leads to pain slightly above the connection point. While working with a physical therapist is the best way to learn exercises and stretches that will be beneficial for you, the following stretches target the gastrocnemius and soleus muscles and have been found to relieve pain for many patients. 

Gastrocnemius Stretch

The gastrocnemius makes up a portion of the Achilles tendon. When stiff, this muscle can be associated with Achilles tendonitis symptoms. To stretch this muscle, follow these steps:

  1. Strand straight in front of a wall or other stable surface. 
  2. Place the hands on the wall and your feet hip-width apart.
  3. Step the affected foot backward while keeping both feet flat on the ground.
  4. Bend the front knee gently while keeping the back leg straight and the back foot flat.
  5. Lean toward the wall so that a stretch is felt alongside the calf of the back leg.
  6. Hold for 30 seconds to 1 minute and repeat 3-5 times. Repeat throughout the day as needed.

Soleus Stretch

The soleus muscle is another muscle associated with the Achilles tendon. To improve the flexibility of this muscle and reduce pain, follow these steps:

  1. Strand straight in front of a wall or other stable surface. 
  2. Place the hands on the wall and your feet hip-width apart.
  3. Step the affected foot backward while keeping both feet flat on the ground.
  4. Bend the back knee gently while keeping the foot flat on the ground.
  5. Lean toward the wall so that a stretch is felt alongside the lower portion of the calf of the back leg.
  6. Hold for 30 seconds to 1 minute and repeat 3-5 times. Repeat throughout the day as needed.
