Boutonniere Deformity

Boutonniere deformity is a deformity of the finger joint that can occur for several reasons. To be classified as this deformity, the finger joint must meet two criteria: 1) The finger is bent at the middle joint (or the first joint in cases of the thumb) and 2) the finger or thumb shows a backward bend at the end of the joint.

One of the ways in which this deformity occurs is when the tendon on the back of the finger or thumb is cut. Another cause of Boutonniere Deformity is when the tendon on the back of the finger or thumb is weakened or torn from an injury or illness, such as rheumatoid arthritis. This leads to the joint showing a bent position and causes the small tendons on the side of the dinger to slide downwards towards the palm side of the finger. This causes an aggravated bend to appear at the finger’s middle joint due to the pull of the tendons. From this, the end joint of the finger can experience more tension, leading to the backward bend of the joint.

Signs of Boutonniere Deformity

Other than the characteristic bends in the finger, the middle joint in the finger may be extremely difficult to straighten. The deformity tends to gradually present and worsen over time, making it exceptionally important to seek treatment early as soon as stiffness or slight deformities develop. Your certified hand therapist will work with you to alleviate the bend in the fingers and build a rehabilitation plan to prevent further deformity.

Treatment of Boutonniere Deformity

Boutonniere Deformity can be treated in many ways, one of which is splinting the finger. This is especially effective if there is a tendon injury and shows the best results if used soon after the tendon is damaged. If the deformity is the result of rheumatoid arthritis, splinting will not give the same results. This type of deformity is often treated by surgical methods, such as repairing the tendon. However, normal positioning in the joints is not always restored completely, and a certified hand therapist can work with you to develop the best rehabilitation plan based on your specific condition and treatment options.
